Medication Information:
Should your child need medication during school hours a signed Parent Request and Physician’s Order form must be submitted with the medication. (Download Form as Word Document or Download Form as PDF file.) This includes all medications, both prescription and over the counter, such as ibuprofen or acetaminophen. Medication will not be accepted at the school without this form signed by both the parent and the physician. The medication container must have the pharmacy label attached with the correct administration information. Medication should not be brought in by students. Parents must bring the medication to the office and sign in the medication. Attention Parents of Rising 6th Graders - North Carolina law now requires a TDaP (tetanus/diptheria/pertussis booster vaccine for entry into 6th grade.
